29 CFR 1926.451(b)(2): Except as provided in paragraphs (b)(2)(i) and (b)(2)(ii) of this section, each scaffold platform and/or walkway was not at least 18 inches (46 cm) wide:    On the east side of the construction site, employees engaged in framing work on a residential home from single pole scaffolds, worked from a platform that was not 18 inches wide.  The employees were exposed to a fall hazard of approximately 18 feet to the ground below.

On the east side of the construction site, the employer did not ensure visible defects in scaffolds were immediately repaired, replaced, or removed from service until repaired, exposing employees to a fall hazard of approximately 18-feet to the concrete or ground below.

29 CFR 1926.451(g)(1)(vii): Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section.   On the east side of the construction site, employees working from a pole scaffold without fall protection system while installing framing material on the exterior of a residential home.  The employee was exposed to a fall hazards of approximately 18 feet to the ground below.
