29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems: (a) Jobsite, 3462 New Germany Road, Ebensburg, PA: On or about June 18, 2020 employees were working on a 10/12 pitch roof and were not protected from falling approximately 10-14 feet to the ground. No guardrails, safety nets or personal fall arrest systems were in use to prevent falls.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards: (a) Jobsite, 3462 New Germany Road, Ebensburg, PA: As of June 18, 2020 the employer had not provided training to employees on how to recognize fall hazards at the jobsite and what procedures and methods could be used to protect oneself from falls.
