29 CFR 1926.501(b)(13):Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system:  32 Dorchester St., Pittsfield, MA: On or about June 17, 2020, two employees were observed roofing this residence, more than 18 feet above the ground below without any fall protection.   Specific abatement documentation that this violation has been corrected must be provided within 10 days of the abatement in accordance with 29CFR1903.19(d)(1).

29 CFR 1926.1053(b)(1): When portable ladders are used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access:  32 Dorchester St., Pittsfield, MA: On or about June 17, 2020, two employees were observed using an aluminum extension ladder to access the roof top work location that did not extend 3 feet past that upper landing surface.  Specific abatement documentation that this violation has been corrected must be provided within 10 days of the abatement in accordance with 29CFR1903.19(d)(1).
