29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501(b).  Worksite: The employer did not ensure that the employee who was required to work from the roof of the residence while installing exterior insulation was protected from fall hazards above 6 feet by the use of guardrails, safety nets or a personal fall arrest system.

29 CFR 1926.1053(b)(7): Ladder(s) that were unsecured or not provided with slip-resistant feet were used on slippery surfaces:  Worksite: The employer allowed an employee to gain access to the roof by the use of a 10-foot aluminum ladder that was disassembled from an aluminum extension ladder, and the 10-foot aluminum ladder was placed on concrete surface without being secured or provided with slip-resistant feet.  On or about 06/19/2020, the above 10-foot aluminum ladder was displaced while an employee was working from the ladder, causing the employee to fall from the ladder.

Worksite: The employer did not ensure that the employees who were working from two (2) separate ladders that were disassembled from an aluminum extension were trained by a competent person in the proper procedures to minimize the hazards associated with ladders.
