29 CFR 1926.453(b)(2)(v): A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift: On or about June 18, 2020, at 20175 Eva St, Montgomery, TX on the North side of the building, employees working from a Snorkel Model TB42 aerial lift were exposed to a fall/ejection hazard while performing framing activities without the use of a body belt with lanyard, attached to the boom or basket.

29 CFR 1926.503(a)(2): The employer did not assure that each employee exposed to fall hazards was trained by a competent person qualified in the areas specified in 29 CFR 1926.503 (a)(2)(i) through (viii) On or about June 18, 2020, and at times prior thereto, at 20175 Eva St, Montgomery, TX, employees were allowed to perform framing work from an aerial lift basket without receiving fall protection training.
