Corp. dba Wendy's Store #45, 6156 Covington Hwy., Lithonia, GA - (Office/Electrical Room) - Employees were exposed to electrical hazard. An employee was engaged in work maintenance activities related to changing out the 480V circuit breaker. The employer did not ensure that the equipment was placed in an electrically safe work condition by not insuring locks and tags were placed on each disconnecting means used to deenergized the circuit and/or equipment being worked on, on or about June 15, 2020.

Calhoun Mgmt. Corp. dba Wendy's Store #45, 6156 Covington Hwy., Lithonia, GA - (Office/Electrical Room) - Employees were exposed to electrical hazard. An employee was engaged in work maintenance activities related to changing out the 480V circuit breaker. The employer did not ensure that the equipment has been verified in that the circuit elements and equipment parts were deenergized before changing out the circuit breakers and/or performing repairs on electrical equipment, on or about June 15, 2020.
