At a worksite located at or near 1311 - 1327 Gator Bowl Boulevard in Jacksonville, Florida, on or about June 12, 2020, an employee sustained a work related injury that resulted in hospitalization. The hospitalization occurred on June 16, 2020. The employer became knowledgeable of the employee's hospitalization on Jun 18, 2020, but did not report the hospitalization to OSHA until June 23, 2020, which was not within 24 hours of the in-patient hospitalization.
