29 CFR 1926.502(d)(17): "Personal fall arrest systems." The attachment point of the body belt shall be located in the center of the wearer's back. The attachment point of the body harness shall be located in the center of the wearer's back near shoulder level, or above the wearer's head. a. On or about June 24, 2020, for the 7215 Monarch Street, Canal Winchester, Ohio worksite, two roofing workers utilized a fall arrest system for protection against falling that included a "mountain climbing" style body belt and related attachment point devices that were not located in the center of the back.

29 CFR 1926.503(a)(1): The employer shall provide a training program for each employee who might be exposed to fall hazards. The program shall enable each employee to recognize the hazards of falling and shall train each employee in the procedures to be followed in order to minimize these hazards. a. On or about June 24, 2020, the employer had not provided a training program for roofing construction activities covered by this part. The employer did not have a training program in recognition of fall hazards and training in procedures to follow, such as the use of equipment approved for fall protection use in a fall arrest system.
