29 CFR 1926.100(a): Employees were not protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns: On or about June 24, 2020, at an excavation site located in a field on the corner of Route 66 and Sara Road in Oklahoma City, OK, an employee was allowed to work in a trench without a hard hat, exposing the employee to being struck by the hoisted/falling material.

29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652(c). On or about June 24, 2020, at an excavation site located in a field on the corner of Route 66 and Sara Road in Oklahoma City, OK, an employee was allowed to work in a trench more than 5 feet deep that did not have adequate protective systems in place to protect employees from cave-ins, exposing employees to crushed-by hazards.
