29 CFR 1926.416(e)(1): Worn or frayed electric cords or cables were used:  At the jobsite at 4945 Old K-7 Highway in Shawnee, Kansas, the Milwaukee portable drill used to mix mortar had a damaged electrical cord in that the outer cable sheath had deteriorated, exposing electrical wiring, exposing employees to an electrical shock hazard.

29 CFR 1926.451(c)(2): Supported scaffold poles, legs, posts, frames, and uprights did not bear on base plates and mud sills or other adequate firm foundation:  At the jobsite at 4945 Old K-7 Highway, Shawnee, Kansas, employee were exposed to fall and struck-by hazards, while conducting masonry activities, in that one leg on the frame of the scaffolding did not have a mud sill.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used.  Crossbraces were used as a means of access.  At the jobsite at 4945 Old K-7 Highway, Shawnee, Kansas, while conducting masonry activities, employees were exposed to fall hazards, approximately 10 feet above the ground below, in that no access or egress was available for employees.  Employees were using the scaffolding frames and crossbraces, for access and egress.
