29 CFR 1926.20(b)(2): The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person(s) 888 Manchester Road, Glastonbury, CT: On or about June 19, 2020, an employee was injured while operating an RT 65 Rear Tine Tiller that was modified from its original configuration. The employee who was injured was working his second day on his own after having been with trainers for approximately 35 working days. While operating an RT 65 Rear Tine Tiller, the employee hit a large gauge piece of steel cable that obstructed the forward progress of the machine. The employee was injured when he attempted to clear the obstruction.

29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury: 888 Manchester Road, Glastonbury, CT: On or about June 19, 2020, an employee who suffered a thumb amputation while operating a modified RT 65 Rear Tine Tiller had not been trained on how to safely shut down the machine prior to clearing obstructions.

29 CFR 1926.300(b)(2): Moving part(s) of equipment exposed to contact by employees or which otherwise created a hazard, were not guarded. 888 Manchester Road, Glastonbury, CT: On or about June 19, 2020, the Model RT 65 Rear Tine Tiller that was modified to be used as an Invisible Fence installation machine, exposed an employee to amputation hazards. The area that originally housed the tines was modified by removal of the guarding and the tines and replaced with an unguarded circular saw type blade.
