29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system:  The employer is failing to protect employees from fall hazards when conducting roofing operations on a residential structure. This was most recently documented on or about June 25, 2020 at 5000 Randolph Street, Lincoln, Nebraska when employees were observed working on a residential structure with a roof pitch greater than 4:12. The employees where installing shingles while working at a height greater than 6 feet above the ground without fall protection.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  The employer is failing to protect employees from fall hazards associated with using ladders during residential construction. This was most recently documented on or about June 25, 2020 at a residential construction jobsite located at 5000 Randolph Street, Lincoln, Nebraska. The ladder used by the employees to access the upper landing surface of the garage was extended approximately two feet above the eve.

29 CFR 1926.1053(b)(9): The area around the top and/or bottom of (a) ladder(s) was not kept clear:  The employer is failing to protect employees from trips and falls associated with roof debris collecting on the ground at the base of a ladder. This was most recently documented on or about June 25, 2020 at 5000 Randolph Street, Lincoln, Nebraska when employees were observed tearing off old shingles on a residential structure causing old shingling and debris to collect at the base of two extension ladder used to access the roof.
