29 CFR 1910.132(d)(2): The employer did not verify, through a written certification, that the required workplace hazard assessment had been performed: Cooper Hatchery, Inc. dba Cooper Farms Cooked Meats located at 6793 U.S. Route 127 N., Van Wert, Ohio 45891: On or about June 12, 2020, the employer had not verified, through a written certification, that the required workplace hazard assessment had been performed to determine if hazards such as SARS-CoV-2, the virus that causes COVID-19, were present that necessitated the use of PPE, such as face shields, in order to protect employees who had contact with suspected or confirmed COVID-19 positive coworkers when performing pre-shift COVID-19 health assessments.
