29 CFR 1926.503(a)(1):The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards: Site, Roof Southeast side: On 06/23/2020 while in the process of installing sheathing on a roof with a 6:12 pitch, 3 employees were exposed to falls approximately 44 feet 8 inches in that, a training program that enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize or abate these hazards.

29 CFR 1926.1060(a):The employer did not provide a training program for each employee using ladders and stairways, as necessary, which would train each employee in the procedures to be followed to minimize hazards related to ladders and stairways: Site, Roof Southeast side: On 06/23/2020, the employer failed to provide ladder safety training to employees tasked with installing sheathing on a 6:12 pitched roof. Employees removed and/or covered over the access to the ladder, and were exposed to falls of 10 ft. while climbing down the edge of the roof to the fourth floor.
