On or about 6/23/2020 at 5310 Matt Highway Cumming, GA. where the employer failed to ensure that electrical extension cords were not used in a "Daisy chain" configuration, exposing workers of all trades to 120 V electrical shock and burn hazards where there was an extension cord, powering a pig tail adapter, powering an extension cord, powering an extension cord, powering a Makita circular saw.
