29 CFR 1910.134(a)(2): On or about 6/11/2020, the employer did not provide appropriate respirators when necessary to protect employee health: a) Hackensack Meridian Jersey Shore University Medical Center located at 1945 Route 33, Neptune NJ: On or about 6/11/2020, employees engaging in direct patient care of symptomatic Patients Under Investigation (PUI) for COVID-19, were not provided with respirators for protection against aerosolized SARS-CoV-2 virus. At a minimum, a N95 NIOSH-certified filtering facepiece respirator is necessary to protect employees from aerosolized SARS-CoV-2 virus. ABATEMENT CERTIFICATION AND DOCUMENTATION REQUIRED FOR THIS ITEM

29 CFR 1904.4(a) The employer did not record each work-related illness case that resulted in general recording criteria on the OSHA Form 300 or equivalent: a) Hackensack Meridian Jersey Shore University Medical Center located at 1945 Route 33, Neptune NJ Booker 3: Employer failed to timely record two work-related illnesses that met the general recording criteria on the OSHA Form 300 or equivalent On or about 6/11/2020, a Unit Secretary was confirmed as positive for COVID-19 and was absent from work for a period of forty-three days due to this illness. The employer did not record the lost time of this employee on the OSHA 300 log or its equivalent. ABATEMENT CERTIFICATION AND DOCUMENTATION REQUIRED FOR THIS ITEM
