Cornerstone Managed Properties, LLC,, worksite located at 1809 Brownstone Blvd., Toledo, Ohio 43614: On or about June 26, 2020, the employer did not ensure that a worker operating an Genie Model #S-40, S/N 729810 aerial lift basket connected a lanyard to the basket while performing soffit painting on an apartment building there by exposing employees to a fall hazard of approximately 18 feet.
