29 CFR 1926.52(a): Employees were exposed to carbon monoxide at concentrations above those specified in the Threshold Limit Values of Airborne Contaminants for 1970 of the American Conference of Governmental Industrial Hygienists of 50 parts per million (ppm) as an 8-hour Time Weighted Average (TWA) concentration:     D. Howe and Sons Construction, Inc., Berry Global Group, Inc. Production Floor located at 1075 Hemlock Road, Morgantown, PA 19543: The Employer controlled operations of Miller Bobcat 250 welding generators indoors to weld on crane rails and bar joist, creating exposures of carbon monoxide (CO) to employees of Berry Global Group, Inc. who were conducting machine operations and packing:      a)   During a 630 minute occupational exposure at a mean calculated 108.4 ppm CO level.  The percentage carboxyhemoglobin (%COHb) in the employee's blood reached a calculated peak level of 14.4%.  Atmospheric pressure, height, weight, activity levels, tobacco smoking behavior, and other factors were taken into account.  The 8-hour TWA exposure was 108.4 ppm CO, which was 2.17 times the OSHA PEL of 50 ppm.        b)   During a 600 minute occupational exposure at a mean calculated 230.8 ppm CO level.  The percentage carboxyhemoglobin (%COHb) in the employee's blood reached a calculated peak level of 26.3%.  Atmospheric pressure, height, weight, activity levels, tobacco smoking behavior, and other factors were taken into account.  The 8-hour TWA exposure was 230.8 ppm CO, which was 4.62 times the OSHA PEL of 50 ppm.        c)   During a 630 minute occupational exposure at a mean calculated 158.5 ppm CO level.  The percentage carboxyhemoglobin (%COHb) in the employee's blood reached a calculated peak level of 19.7%.  Atmospheric pressure, height, weight, activity levels, tobacco smoking behavior, and other factors were taken into account.  The 8-hour TWA exposure was 158.5 ppm CO, which was 3.17 times the OSHA PEL of 50 ppm.        d)   During a 667 minute occupational exposure at a mean calculated 94.7 ppm CO level.  The percentage carboxyhemoglobin (%COHb) in the employee's blood reached a calculated peak level of 12.8%.  Atmospheric pressure, height, weight, activity levels, tobacco smoking behavior, and other factors were taken into account.  The 8-hour TWA exposure was 94.7 ppm CO, which was 1.89 times the OSHA PEL of 50 ppm.        e)   During a 675 minute occupational exposure at a mean calculated 159.3 ppm CO level.  The percentage carboxyhemoglobin (%COHb) in the employee's blood reached a calculated peak level of 19.4%.  Atmospheric pressure, height, weight, activity levels, tobacco smoking behavior, and other factors were taken into account.  The 8-hour TWA exposure was 159.3 ppm CO, which was 3.19 times the OSHA PEL of 50 ppm.        f)    During a 370 minute occupational exposure at a mean calculated 261.5 ppm CO level.  The percentage carboxyhemoglobin (%COHb) in the employee's blood reached a calculated peak level of 27.5%.  Atmospheric pressure, height, weight, activity levels, tobacco smoking behavior, and other factors were taken into account.  The 8-hour TWA exposure was 201.6 ppm CO, which was 4.03 times the OSHA PEL of 50 ppm.        g)   During a 690 minutes occupational exposure at a mean calculated 149.9 ppm CO level.  The percentage carboxyhemoglobin (%COHb) in the employee's blood reached a calculated peak level of 18.9%.  Atmospheric pressure, height, weight, activity levels, tobacco smoking behavior, and other factors were taken into account. Production Floor located at1075 Hemlock Road, Morgantown, PA 19543: Engineering and administrative controls were not implemented to achieve compliance when employees were overexposed to carbon monoxide (CO).  The Employer controlled operations of Miller Bobcat 250 welding generators indoors to weld on crane rails and bar joist, creating exposures of carbon monoxide (CO) to employees of Berry Global Group, Inc. who were conducting machine operations and packing:    a)      During a 630 minute occupational exposure at a mean calculated 108.4 ppm CO level. The percentage carboxyhemoglobin (%COHb) in the employee's blood reached a calculated peak level of 12.8%.  Atmospheric pressure, height, weight, activity levels, tobacco smoking behavior, and other factors were taken into account.  The 8-hour TWA exposure was 94.7 ppm CO, which was 1.89 times the OSHA PEL of 50 ppm.      e)      During a 675 minute occupational exposure at a mean calculated 159.3 ppm CO level.  The percentage carboxyhemoglobin (%COHb) in the employee's blood reached a calculated peak level of 19.4%.  Atmospheric pressure, height, weight, activity levels, tobacco smoking behavior, and other factors were taken into account.
