29 CFR 1910.119(d)(3)(ii): The employer did not document that equipment complies with recognized and generally accepted good engineering practices:  (a) On or about 07/08/2020 - Engine Room, employees were exposed to ammonia hazards due to the door between the Engine Room and Maintenance Shop would not stay closed, was not tight-fitting, did not swing in the direction of egress to leave the Engine Room, and was not provided with panic-type hardware as specified in International Institute of Ammonia Refrigeration (IIAR) ANS/IIAR 2-2014 section 6.10.2.  (b) On or about 07/08/2020 - Engine Room, employees were exposed to ammonia hazards due to the door between the Engine Room and the outside was not provided with panic-type hardware as specified in International Institute of Ammonia Refrigeration (IIAR) ANS/IIAR 2-2014 section 6.10.2.  (c) On or about 07/08/2020 - Engine Room, employees were exposed to ammonia hazards due to the Engine Room ammonia detector not activating an alarm that reported to a monitored location as specified in International Institute of Ammonia Refrigeration (IIAR) ANS/IIAR 2-2014 section 6.13.1.2.  (d) On or about 07/08/2020 - Ice Box Room, employees were exposed to ammonia hazards due to the Ice Box Room contained refrigeration equipment such as, but not limited to, vessel VS-108 and valve assemblies and did not have an ammonia level 1 detection and alarm as specified in International Institute of Ammonia Refrigeration (IIAR) ANS/IIAR 2-2014  section 7.2.3.
