29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets: On or about June 23, 2020, employees were exposed to a struck by hazard while performing framing activities on a new residential home located at 29949 Persimmon Road, Westlake, Ohio 44145. There were employees on the roof working with tools and materials that could have fallen and struck the employee working on the ground and the employer did not ensure that the ground employee was wearing a protective helmet.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): On or about June 23, 2020, employees were exposed to a fall of approximately 21 feet while performing framing activities on a new residential home located at 29949 Persimmon Road, Westlake, Ohio 44145. The employees were walking the top plate of the home and the employer did not ensure that they were using fall protection.
