29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems: (a) Top roof - On or about July 29, 2020 the employer did not ensure the employees working on a roof greater than 6 feet high were utilizing a means of fall protection, exposing them to fall hazards.

29 CFR 1926.503(c): The employer did not retrain affected employees who already had been trained but demonstrated inadequate understanding and skill required by paragraph (a) of this section:Note: Circumstances where retraining is required include, but are not limited to, situations where (1) Changes in the workplace render previous training obsolete; or (2) Changes in the types of fall protection systems or equipment to be used render previous training obsolete. (a) Top of roof - On or about June 29, 2020 the employer did not provide retraining for employees that were not utilizing their personal fall arrest system correctly, exposing them to fall hazards.
