29 CFR 1926.416(a)(1):Employees were permitted to work in proximity to electric power circuits and were not protected against electric shock by de-energizing and grounding the circuits or effectively guarding the circuits by insulation or other means: Location - 377 Putnam Ave., Cambridge, MA, 02139 On or about June 25, 2020, employees were exposed to electrical hazards, while chipping concrete near a 13kV live electrical line. Employer did not de-energize or provide effective guarding.

29 CFR 1926.416(a)(3):Employer did not post or maintain proper warning signs at locations where the performance of the work could bring a person, tool, or machine into physical or electrical contact with an energized electrical power circuit: Location - 377 Putnam Ave., Cambridge, MA, 02139 On or about June 25, 2020, the employer did not warn employees about the hazards or the proper protective measures to be taken when they worked in close proximity to 13 kV live electrical lines.
