All protruding reinforcing steel, onto and into which employees could fall, did not have guards to eliminate the hazard of impalement. a) Mammoth Cave, Grand Ave Tour, on or about 29 Jun, 2020, workers preparing to pour concrete were exposed to unprotected rebar exposing them to potential impalement, cuts, and or lacerations.
