29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  On or about July1, 2020, employees were exposed to a struck by hazard while performing roofing activities on a commercial building located at 483 Prospect, Berea, Ohio 44017.  Employees were working on upper levels with tools and materials that could have fallen on lower employees and the employer did not ensure that the lower employees were wearing protective helmets.

29 CFR 1926.102(a)(1): Employees were not provided with eye and face protection equipment when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:  On or about July1, 2020, employees were exposed to a struck by hazard while performing roofing activities on a commercial building located at 483 Prospect, Berea, Ohio 44017.  The employees were using air powered nail guns and the employer did not ensure that they were wearing protective safety glasses.

The employees were working on all areas of the roof and the employer did not ensure that they were using fall protection.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  On or about July1, 2020, employees were exposed to a fall of approximately 15 feet while performing roofing activities on a commercial building located at 483 Prospect, Berea, Ohio 44017.  The employees were using an extension ladder to access the roof and the employer did not ensure that it extended three feet above the landing area.
