The employer did not provide, and ensure each employee uses, a safe means of access and egress to and from walking-working surfaces: Dry storage forklift ramp location. On or about June 30, 2020, and at times prior thereto, employees were exposed to struck-by hazards while accessing the forklift ramp that had damage to its side rails and concrete.
