IN ACCORDANCE WITH 29 CFR 1903.19(d), ABATEMENT CERTIFICATION IS REQUIRED FOR THIS VIOLATION (using the CERTIFICATE OF CORRECTIVE ACTION WORKSHEET) AND IN ADDITION DOCUMENTATION DEMONSTRATING THAT ABATEMENT IS COMPLETE MUST BE INCLUDED WITH YOUR CERTIFICATION. 29 CFR 1910.180(h)(3)(ix): Outriggers were not used when required to handle a load at a given radius a) 27 Surrey Lane, Tinton Falls NJ: On or about 07/01/20, workers were exposed to injury from a crane collapse when an employee was attempting to stow the extended boom of the crane without the front outriggers deployed/set as per the crane manufacturer's requirements. NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm in that employees were exposed to being struck by hazards where equipment repairs and modifications had been made without the manufacturer's approval: a) 27 Surrey Lane, Tinton Falls NJ: On or about 07/01/20, and times prior thereto, employees were exposed to struck by hazards when the boom of the company crane was being stowed while its outriggers were not fully extended and set as per the manufacturer's requirements. NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
