In the facility at 24 E Brookfield Rd, North Brookfield, MA, on and before, 07/01/2020, the employer did not ensure that employees working in the restoration area and required to wear a half face respirator were fit tested initially.  The employer provided different brand of half face respirators throughout the year (e.g. North, 3M), and did not fit test employees prior to issuing a different brand/size.  The employer did not set up an annual fit testing schedule for employees required to wear a half face respirator.
