29 CFR 1926.20(b)(2): The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person(s). Location - 191 Strand Way, Winthrop, MA: On or about 06/23/2020, a frequent and regular inspection of the job site was not performed to protect workers from the following but not limited to hazards; 1. Fall hazards of up to approximately 30-feet, while working near and around window openings without fall protection; 2. Fall hazards greater than 5-feet, 10-inches, while entering into the house from a closed A-frame portable ladder to gain access to the house.
