On or about 6/26/2020, at jobsite located at 13807 NW 4 Street, Sunrise, Florida, an employee suffered a work-related injury that resulted in an in-patient hospitalization on 6/26/2020; the employer was informed of the hospitalization on 6/27/2020, and reported said hospitalization on 7/1/2020.
