29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels and 50-feet (15.25 m) wide or less, was not protected from falling by guardrail systems, safety net systems:      On or about 6/24/2020, the employer did not ensure appropriate fall protection was implemented for employees performing commercial roofing operations on a 4:12 pitch roof that was greater than 50 feet wide:     Employees were installing metal roof sheeting were exposed to fall hazards associated with an approximately twelve (12) foot fall and no fall protection was used.

29 CFR 1926.1053(b)(1): Portable ladders are used for access to an upper landing surface and the ladder side rails do not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access:      On or about 6/24/2020, the employer did not ensure portable ladders were constructed and placed so three (3) feet or more was extended above the upper landing.  Workers performing commercial roofing operations were exposed to falling approximately twelve (12) feet while using an extension ladder to ascend/descend from the ground level to the roof.
