On or about June 12, 2020, a long fork electric pallet jack powered industrial truck (PIT) collided with other forklift truck while approaching the staging area at Aisle A1 due to the vision was obstructed by stored material (wooden pallets), on coming and entering traffic. The driver/operator of the PIT sustained injuries.
