29 CFR 1910.28(b)(6)(i): (i) Each employee less than 4 feet (1.2 m) above dangerous equipment is protected from falling into or onto the dangerous equipment by a guardrail system or a travel restraint system, unless the equipment is covered or guarded to eliminate the hazard.  On or about July 13, 2020 a compactor located in the dock area of Building 1 was observed to have a guardrail measuring only 33 inches above the dock surface.  This exposed employees to falls into the compactor, which could result in injury.

29 CFR 1910.29(b)(2)(iii): (iii) Intermediate vertical members (such as balusters) are installed no more than 19 inches (48 cm) apart:  On or about July 13, 2020 the space between a compactor and the existing guardrail was found to be 25 inches wide and was 52 inches in height from the dock platform to the ground. This exposed employees to falls due to the lack of a guardrail in this space.

29 CFR 1910.335(a)(1)(i): Employees working in areas where there were potential electrical hazards were not provided with electrical protective equipment that was appropriate for the specific parts of the body that needed to be protected and for the work being performed:  On or about July 13, 2020 an employee was missing his balaclava hood that is part of the arc flash PPE. The absence of an arc flash balaclava hood exposed the employee to burns.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  a) On or about July 7, 2020 a belt sander in the maintenance shop area of Building 2 was found to be missing guarding at the bottom ingoing nip point. This exposed employees to hand, arm, and eye injuries associated with the sanding process.    b) On or about July 13, 2020 a Heidenreich and Harbeck Lathe in Building 1 was found to be missing a guard over the lead screw. This exposed employees to hand and arm injuries associated with the operation of this machine.   c) On or about July 13, 2020 a horizontal band saw located in the Tooling Department of Building 1 was not provided with a guard for the unused portion of the blade. This exposed employees to hand injuries from operating.   d) On or about July 13, 2020 a Okuma LB15 Lathe was found to have an interlock that allowed the doors to open approximately 4 1/2 inches. This opening was too wide and exposed employees to hand and arm injuries if they reached in during operation.

This exposed employees to eye and face injuries from debris.   b) On or about July 13, 2020 an air nozzle in Building 1 near the Maching G400 machine was found to be at 35 p.s.i.. This exposed employees to eye and face injuries from debris.   c) On or about July 13, 2020 an air nozzle in Building 1 near the Fastums System (1E03 area) was found to be at 50 p.s.i.. This exposed employees to eye and face injuries from debris.

29 CFR 1910.303(g)(1)(i)(B): For electric equipment operating at 600 volts, nominal, or less to ground, and likely to require examination, adjustment, servicing, or maintenance while energized, the width of the working space in front of electrical equipment was less than the width of the equipment or 762 mm (30 inches), whichever was greater :  On or about July 13, 2020 an electric panel box in Building 1 was found to be obstructed by piping. The width was found to be 18 1/2 inches and the depth was found to be 8 1/2 inches.  Employees were exposed to possible prolonged exposure to electrical sources due to the electric panel box being inaccessible.
