29 CFR 1910.28(b)(7):  The employer did not ensure that each employee on a walking-working surface near an opening, including one with a chute attached, where the inside bottom edge of the opening is less than 39 inches (99 cm) above that walking-working surface and the outside bottom edge of the opening is 4 feet (1.2 m) or more above a lower level is protected from falling by the use of: (i) Guardrail systems; (ii) Safety net systems; (iii) Travel restraint systems; or, (iv) Personal fall arrest systems.      Employees were exposed to fall hazards. Employees working in and around the lime pit during vacuuming activity were exposed to fall hazards. The employer did not ensure the employees were protected from falling by use of a fall protection system. The fall distance was approximately 7 feet 2 inches from the pit opening to the pit floor below.

Employees required to wear tight fitting, air filtering cartridge respirators while vacuuming lime pebble and granular quicklime from a lime pit were exposed to inhalation hazards. 29 CFR 1910.146(f)(7): The entry permit that documented compliance and authorized entry to a permit space did not identify the hazards of the permit space to be entered:    Employees were exposed to chemicals hazards. Employees vacuuming lime pebble and granular quicklime  from a lime pit were exposed to dermal and inhalation hazards. The confined space entry permit for the L. G. Everist lime pit, located at 8440 Gibbs Rd. in Kansas City, KS 6611 did not identify lime exposure as a potential hazard.
