29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:    496 Purgatory Rd, Whitinsville, MA: On July 8, 2020 and at times prior, employees did not utilize the energy control procedure when they reached into the splitting/sawing area to unjam a firewood processor machine.  On July 3, 2020, an employee unjamming a processor machine without it being locked out, received a severe arm injury.

29 CFR 1910.147(c)(6)(ii): The employer did not certify that periodic inspections of the energy control procedures had been performed.  The certification shall identify the machine or equipment on which the energy control procedure was being utilized, the date of the inspection, the employees included in the inspection, and the person performing the inspection:    496 Purgatory Rd, Whitinsville, MA: On July 8, 2020 at the start of the inspection, the employer had no documents to certify the accomplishment of the annual periodic inspection that ensured each procedure was inspected for inaccuracies during an actual energy control task.  Additionally, the employer had no documentation for the review, between the inspector and each authorized employee, of that employee's responsibilities each energy control procedure that they were authorized to use.
