29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:  At the time of the hospitalization on 6/24/2020, the employer had not implemented an energy control program that would have prevented operation of the HMC HC 40 HD Head Saw while an employee was performing maintenance. The employer had no procedure to isolate the energy source to prevent the unexpected operation of the Head Saw and Log Carriage.    Abatement certification and abatement documentation is required for this violation.  The documentation should include written verification of abatement, applicable measurements or monitoring results, and photographs or videos which you believe will be helpful.  The abatement certification sheet is enclosed with the citations.
