29 CFR 1926.100(a): The employer did not ensure that employees working in areas where there is a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns, were protected by protective helmets. This violation occurred on July 10, 2020, where employees working below employees working on steel erection above, were exposed to being struck by falling materials and/or tools from more than 14 feet above.

29 CFR 1926.451(c)(2)(v): Fork-lifts shall not be used to support scaffold platforms unless the entire platform is attached to the fork and the fork-lift is not moved horizontally while the platform is occupied. This violation occurred on July 10, 2020, where employees working on an elevated work platform were exposed to a fall of approximately 12 feet to the concrete below. Platform was not secured to the forks.

29 CFR 1926.1053(b)(1): The employer did not ensure that when portable ladders are used for access to an upper landing surface, the ladder side rails shall extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access; or, when such an extension is not possible because of the ladder's length, then the ladder shall be secured at its top to a rigid support that will not deflect, and a grasping device, such as a grabrail, shall be provided to assist employees in mounting and dismounting the ladder. In no case shall the extension be such that ladder deflection under a load would, by itself, cause the ladder to slip off its support. On or about July 10, 2020, employees used an extension ladders that were not secured or at least 3 feet above the landing when accessing and for egress the steel. Employees were exposed to a fall of more than 14 feet to the concrete below.
