29 CFR 1926.95(c): All personal protective equipment was not of safe design and construction for the work to be performed: On or about July 10, 2020, and at times prior thereto, employees installing concrete hollow core planks were exposed to a fall hazard of greater than 6 feet while using a Guardian Velocity Retractable self-retracting lifeline (SRL), Model 42002, that was not manufactured and designed to be used on a leading edge.
