29 CFR 1910.147(c)(4)(ii)(B): The energy control procedures did not clearly and specifically outline the steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy. On or about June 16, 2020 and at times prior thereto, all necessary specific steps and equipment for controlling hazardous energy sources were not detailed in the written LOTO procedures for the replacement of components within the Flipover Machines, exposing the employee to the hazards of being crushed and/or struck by the Flipover.
