Located at 5200 Meadowview Lane (lot 64), Hilliard, Ohio: On or about July 13, 2020, the employer failed to ensure that an employee who worked on the roof top of the second floor new build residential home was protected from falling to the ground level.  The employee was exposed to a fall of approximately 18 feet from the roof top eave to the ground level.  Within the past five years, the employer had been cited for the same hazard in a different inspection.  Ricardo Dominguez was previously cited for a violation for this Occupational Safety and Health standard, which was contained in OSHA inspection number 1435093, citation number 2, item number 1 and was affirmed as final order on October 31, 2019, with respect to a workplace located at 5267 Meadowview Lane, Lot 58, Hilliard, Ohio.  Ricardo Dominguez was previously cited for a violation for this Occupational Safety and Health standard, which was contained in OSHA inspection number 1265388, citation number 1, item number 1 and was affirmed as final order on November 07, 2017, with respect to a workplace located at 693 Viola Drive, Lot 1849, Sunbury, Ohio.
