1910.28(b)(3)(i)(A) The employer did not ensure that each employee on a walking-working surface was protected from falling into any hole or pit that was 4 feet (1.2 m) or more above a lower level by a cover or covers: On or about July 13, 2020, on the roof of a maintenance workshop located at 12950 West Little York in Houston, TX, an employee was exposed to a fall hazard when performing maintenance around unguarded skylight(s) that was greater than 4 feet above the lower level, without using hole covers.
