29 CFR 1910.1200(h)(2)(iii): The employer did not provide information to the employees as to the location and availability of the written hazard communication program, and material safety data sheets required by 29 CFR 1910.1200:  (a) Elite Health Partners, LLC, 1725 Skyway Drive, #160, Longmont, CO 80504: On or before July 14, 2020, the employer did not adequately train employees on the location of safety data sheets at the location. This condition exposes employees to chemical hazards in the workplace.

29 CFR 1910.134(d)(1)(iii): The employer did not identify and evaluate the respiratory hazard(s) in the workplace; including a reasonable estimate of employee exposures to respiratory hazards and identification of the contaminant's chemical state and physical form:  (a) Elite Health Partners, LLC, 1725 Skyway Drive, #160, Longmont, CO 80504: On or before July 14, 2020, the employer did not ensure assess employee's exposure to air contaminants in the formulation area. This condition exposed employees to respiratory hazards associated with ethanol, and hydrogen peroxide.

29 CFR 1910.134(e)(1): The employer did not provide a medical evaluation to determine the employee's ability to use a respirator, before the employee was fit tested or required to use the respirator in the workplace:  (a) Elite Health Partners, LLC, 1725 Skyway Drive, #160, Longmont, CO 80504: On or before July 14, 2020, the employer did not ensure that all employees who were told wear a tight-fitting respirator while formulating hand sanitizer were medically evaluated prior to use of the respirator. This condition exposed employees to hazards associated with respirator use.   Note:  The employer may discontinue an employee's medical evaluations when the employee is no longer required to use a respirator.

29 CFR 1910.134(f)(1): The employer did not ensure that employee(s) required to use a tight-fitting facepiece respirator passed the appropriate qualitative fit test (QLFT) or quantitative fit test (QNFT):  (a) Elite Health Partners, LLC, 1725 Skyway Drive, #160, Longmont, CO 80504: On or before July 14, 2020, the employer did not ensure that all employees who were told wear a tight-fitting respirator while formulating hand sanitizer were fit-tested to ensure proper fit. This condition exposed employees to hazards associated with air contaminants including, but not limited to, ethanol, and hydrogen peroxide.    Note:  The employer may discontinue an employee's medical evaluations when the employee is no longer required to use a respirator.
