29 CFR 1926.1053(b)(6): Ladders were used on surfaces which were not stable and level:     On or about July 6, 2020 at the job site located at 1721 Deerwood Cir SE, East Sparta, Ohio 44626; during the delivery of roofing material on a residential unattached garage roof, the employer did not ensure a portable ladder was set up properly by selecting a stable level surface that did not have a slope which allowed for the ladder to kick/slip out as an employee climbed down the ladder.

1926.1053(b)(7)  :Ladders shall not be used on slippery surfaces unless secured or provided with slip-resistant feet to prevent accidental displacement. Slip-resistant feet shall not be used as a substitute for care in placing, lashing, or holding a ladder that is used upon slippery surfaces including, but not limited to, flat metal or concrete surfaces that are constructed so they cannot be prevented from becoming slippery.     On or about July 6, 2020 at the job site located at 1721 Deerwood Cir SE, East Sparta, Ohio 44626; The portable extension ladder set up to gain access to unattached garage roof was setup on a grasses lawn slope. The ladder did not have slip-resistant feet and was not secure at the bottom to prevent accidental displacement and cause ladder slip out while in use.
