29 CFR 1926.451(c)(1)(ii): Guys, ties, and/or braces were not installed according to the scaffold manufacturer's recommendations or at the closest horizontal member to the 4:1 height and repeated vertically at locations of horizontal members every 20 feet (56.1 m) or less thereafter for scaffolds 3 feet ((0.91 m) wide or less:    a) Jobsite, north end of building:  On or about July 15, 2020 and at times prior thereto a single tie-back brace was used to secure a multi-level scaffold system.  The scaffold was 3 feet wide and 5 levels high (between 32 and 35 feet high). The single tie-back was located at approximately 32 feet high from the base of the scaffold. 29 CFR 1926.454(a): The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards:  a) Jobsite, north end of building: On or about July 15, 2020 and at times prior thereto the employer did recognize or acknowledge the hazards of climbing down the cross bracing of a scaffold system.        b) Jobsite, north end of building: On or about July 15, 2020 and at times prior thereto employees did recognize or acknowledge the importance of installing tie-back bracing at the required location(s) on a multi-level scaffold system and the hazards associated with this.
