In the facility at 37 Montcalm St, Chicopee, MA: On and before, 07/16/2020, the employer did not ensure that the access to the back exit located in the storage room/manager's office area of the store was at least 28 inches wide. The exit access area was used for storage of supplies and equipment narrowing the exit route to 12.5 inches at some points along the route and not more than 27" in others.

29 CFR 1910.157(c)(1): The employer shall provide portable fire extinguishers and shall mount, locate and identify them so that they are readily accessible to employees without subjecting the employees to possible injury. In the facility at 37 Montcalm St, Chicopee, MA: On and before, 07/16/2020, the employer had a fire extinguisher next to the exit door and the fire extinguisher was blocked by three stacked cardboard boxes and a snow shovel.
