29 CFR 1910.36(g)(2): Exit access was not at least 28 inches (71.1 cm) wide at all points.     In the facility at 37 Montcalm St, Chicopee, MA: On and before, 07/16/2020, the employer did not ensure that the access to the back exit located in the storage room/manager's office area of the store was at least 28 inches wide.  The exit access area was used for storage of supplies and equipment narrowing the exit route to 12.5 inches at some points along the route and not more than 27" in others.

29 CFR 1910.157(c)(1): The employer shall provide portable fire extinguishers and shall mount, locate and identify them so that they are readily accessible to employees without subjecting the employees to possible injury.  In the facility at 37 Montcalm St, Chicopee, MA: On and before, 07/16/2020, the employer had a fire extinguisher next to the exit door and the fire extinguisher was blocked by three stacked cardboard boxes and a snow shovel.

29 CFR 1910.176(b): Material stored in tiers was not stacked, blocked, interlocked or limited in height so that it was stable and secure against sliding and collapse.  In the facility at 37 Montcalm St, Chicopee, MA: On and before, 07/16/2020, the employer stored cardboard boxes, plastic containers and other materials above head level and did not stack or secure the material properly.  Heavier material was stored on top of lighter material and the material was not secured against sliding or collapsing.
