The employer reported the death to OSHA on July 15, 2020 after being contacted for confirmation based on data from the CMS (Centers for Medicare & Medicaid Services) database showing two staff deaths from Covid-19.  b)  Magnolia Nursing and Rehabilitation located at 1105 N Magnolia Avenue, Luling, TX 78648: On or about June 24, 2020, an employee died of COVID-19.  The employer did not report the work-related fatality to OSHA within 8 hours.
