29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative: Employees were exposed to struck-by and caught-in hazards. Employees operating and/or maintaining the metal roll forming machine were exposed to struck-by and caught-in hazards. The employer did not establish a training program for employees, required to perform maintenance on the metal roll forming machine, on the lockout/tagout program. The metal roll forming machine had electrical and mechanical sources of energy.

This was most recently documented on or about July 20, 2020 at the Metal Panels, Inc. warehouse, located at 8341 Ruby Ave in Kansas City, Kansas 66111. Employee(s) operating and maintaining the roll forming machine were exposed to in-going nip points. The guard(s) protecting employees from exposure to rotating sprockets and chains on the roll forming machine were not affixed to the machine and the sensors provided to ensure that the guard(s) were in place during machine operation had not been inspected to ensure that they were operational. A guard was removed, the roll former continued operation, and an amputation resulted.

This was most recently documented on or about July 20, 2020 at Metal Panels, Inc. warehouse, located at 8341 Ruby Ave in Kansas City, Kansas 66111. Employee(s) operating and maintaining the roll forming machine were exposed to in-going nip points. Employee(s) were exposed to chain and sprockets, which were approximately 4 feet from the floor, when the guard(s) were removed while making adjustments to the profile and/or repair resulting in an amputation.
