(a)  At the Feeder Warehouse construction site, on or about July 15, 2020 and at times prior to, the employer did not obtain written certification from the manufacturer or by any other equivalent entity to be in conformity with all applicable provisions of ANSI A92.2-1969 and section 29 CFR 1926.453 prior to using an aerial lift (Make: JLG Model 800AJ) to lift steel beams supported from beneath the basket, exposing employees to struck-by and fall hazards.
