29 CFR 1926.652(a)(1): Each employee in an excavation is not protected from cave-ins by an adequate protective system designed in accordance with paragraph (b) or (c) of this section. a) On or about July 15, 2020, and at times prior thereto, the controlling contractor did not ensure that its sub-contractor Keller Construction protect each worker by properly sloping the sides or using appropriate protective systems such as a trench box. Employees were drilling epoxy and rebar in preparation for a concrete pour in a trench at a depth of approximately 6.0 feet. Abatement certification AND documentation are required for this item.
