29 CFR 1926.404(f)(6): The path to ground from circuits, equipment, or enclosures was not permanent and continuous:  a) First floor room at the south corner of Building 146, on or about July 14, and at times prior to, an employee was using a Kool Flo, Model SFC1-450B 18" portable fan that had a missing ground pin on the power cord plug, resulting in an electrical hazard.
