29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   Location: Front east side of residential home @ 132 Greaton Road, West Roxbury, MA 02132  On or about 7/18/20 employees were working on a flat roof of a new addition to an existing residential home. Employees were 33 feet above the ground and no fall protection was used.  The Foreman of the company along with other workers were putting down roofing board insulation.  A section of the insulation was over hanging the roof edge 2 feet. The Foreman stepped on the insulation board where it then broke allowing him to fall 33 feet to the ground sustaining serious injuries.
